He joined the M.G.R. Government Film and Television Training Institute in Tamil Nadu after receiving his degree from University College Thiruvananthapuram, and he later began working as an ad editor. He then advanced to editing documentaries and short films before breaking into the industry with the Malayalam film Rathri Mazha, which won five Kerala State Film Awards.

Mahesh Narayanan made his acting debut in 2017, starring in the Indian drama-thriller Take Off, which is based on the 2014 ordeal of Indian nurses in Tikrit, Iraq. The main actors were Parvathy Thiruvothu, Kunchacko Boban, and Fahadh Faasil. It was written by P. V. Shajikumar and Mahesh Narayanan. The film was filmed in a number of places in Kerala and Dubai.
The release day for Take Off was March 24, 2017. With over 40 crores in box office receipts and over 125 days in theatres, the movie was a box office triumph. The movie was shown in competition at a number of international film festivals, such as the International Film Festival of India and the International Film Festival of Kerala.
It won a Special Jury Award at the 48th International Film Festival of India. Five Kerala State Awards were given to the movie, including Mahesh Narayanan’s Best Debut Director Award.
In June 2020, Mahesh Narayanan disclosed that his upcoming work would be the experimental movie C U Soon, starring Fahadh Faasil. Mahesh claimed that rather than being a full-length movie, the film was more of a brief exercise in using an iPhone.
Additionally, it was stated that the movie would be filmed at Faasil’s apartment and would last 60 to 65 minutes (the actual running length was 1 hour and 38 minutes). Despite the Kerala Film Employees Federation of Kerala’s agreement, the Kerala Film Producers Association (KFPA) opposed the filming because of the disruptions caused by the COVID-19 outbreak (FEFKA). Despite this, filming on August 21 was completed without incident.
Critics gave the movie high marks after its OTT debut on Amazon Prime Video. It is the first computer-generated movie in the nation. C U Soon was shown in the screen-life section at the 42nd Moscow International Film Festival.
